Probably that openAL, whatever the hell that is, not familiar with it. Is there any way to run audio without that?
Try changing settings one by one until you find the bad one (with me, it was s_khz "44"; it needed to be "22")
s_initsound 1 is directsound, only s_initsound 2 loads openal. It isn't effecting you in this case.

I'm pretty sure most of (if not all) the sounds used in Quake2 are sampled at 22khz, so using 44khz causes the 22khz sounds to be converted up to 44khz which will often add fuzzy noise to the higher frequencies that you may not notice through monitor speakers. It really just depends on the construction of the speakers (size of the drivers, etc) as to how noticeable it is.
