EGL+Hi-res textures for almost all the graphics (Including models), and every effect jacked up as high as it can go. Note that this is scaled down from 1600x1200 with 8xS FSAA and 16x Anisotropic filtering.

I don't use anything cheap like bright skins or counters, though I do mildly adjust my gamma with Powerstrip due to Quake II being darker than a bum's asshole at midnight on anything but a 3Dfx card. I also have a mixture of QIII and QIV sounds to replace the weapon effects and some other things (Like teleportation, etc), as well as some of the HUD graphics from Adam's PAK.

edit: I also play the Quake 1 soundtrack on shuffle with Winamp whenever I play.

Actually.. it's more WoW thing if anything.. you say gief as in give something. like you want shadow priest in group you say gief spriest
