Have you ever seen, or heard rather, of a pak to get a 'rail hit' sound like in q3a?

i dont think this possible without a new mod

It's a couple lines of code to add a hit sound to the Quake2 game code:
http://jdolan.dyndns.org/trac/browser/quake2world/src/game/default/p_client.c#L286

That funciton was called "player_pain" in stock Quake2, and was empty as of 3.21.  The ATTN_STATIC attenuation constant is used because the sound should be heard only by the attacker, or people chasecam'ing him.
