Whats the difference between vsync and the "sync every frame" option?
I know these console commands affect fps:
gl_ext_pointparameters
gl_swapinterval
gl_ext_swapinterval
gl_ztrick
cl_maxfps
When I switch that "sync every frame" option in the actual options menu, I can't tell what command it affects. Can anyone help?

it effects gl_swapinterval and gl_ext_swapinterval
if you want vsync turned off, both should be set to 0
may also want to check your video card as it my stil force it

Ok. What does "sync ever frame" in options do then?

it controls vsync from within the game
turning it on turns on vsync

I figured it out. It changes gl_finish. It has no effect on gl_swapinterval or gl_ext_swapinterval.

NO, sync every frame will match your monitor refresh rate with the in game frames per second. this only works if first, your game fps is consistantly more than your monitor, and you experiance tearing and other visual artifacts. your game will also be somewhat smoother, since the game is in natual 'sync' with the monitor.

False. The 'sync' in Quake2 video properties is not vsync, it's to do with input device (mouse/keyboard) sync with opengl driver.
Input device sync cvars
set gl_finish "1" //0 if no input lag
Vsync cvars
set gl_ext_swapinterval 1
set gl_swapinterval 0
