Handicap feature, and +noobquad setting

Note: Since the handicap feature doesn't affect gameplay for anyone who chooses not to use it, I figured it was safe to enable on vanilla, mutant, and dm.  (The noobquad feature is not enabled, just handicap.)

Pending Bugs:

- Hulkster unable to set handicap
- Run Checkwinner() on disconnect, timeout, or kick (possibly rewrite tracking highscores instead of using "iswinner" flag)
- New calculation for handicapped players shooting a lower percentage handicapped players.
- NoobQuad not working for DEF on last 2 games.

(WallFly is a bug but is no longer pending LOL)

Same problem with Krushr on Vanilla. Never got feedback while setting handicap.

Could wallfly be eating the command?
