Wireless packet loss (if it is the wireless to blame) is nearly always down to interference.  The most obvious solution is to change the channel (frequency range) that the router operates on using it's config pages.

Netgear routers all come out of the factory set to channel 6 so if you got 3 or 4 local wireless networks and maybe 2 use a Netgear, chances are your going to get interference even if you got a great signal strength.

Not sure what the wireless standards are in US but in UK it's best to use channels 1, 6 or 11 for the router as the rest of the channels share frequencies with each other, sort of overlapping.

Having said that I've seen this 'sawtooth' lag before - I used to get it when there was more than one connection to the server through my router.  Reconnecting one of the clients usually fixed it for a bit but I never really knew what caused it.

Any electrical items, especially things like baby monitors (already mentioned) or cordless phones, or any large sheets of metallic substances in between the router and adapter can also cause a crappy wireless connection.  It may be as simple as moving your equipment about a bit to fix this.
