I found a setting in the nvidia control panel that fixed it. It was the "Threaded optimization" setting, which it describes as:"Allows applications to take advantage of multiple CPUs. Most newer applications should benefit from the Auto-select or On settings. This setting should be turned off for most older applications."I turned it Off and Q2 runs fine now. (One nice thing is their control panel lets you override settings for particular program .exe files. So I just turned it off for r1q2.exe and quake2.exe, and left it "Auto" for the rest of the system.)

do you all have nvidia's & multi-core's? I have an AMD X2 & an ATI but have never experienced these issues. Infact, I tried turning off one CPU last night to see if it made a difference & it didn't.
So where is the 'threaded optimization" thingee? I even did an index search in my Nvidia Control Panel and it ain't there. I went to their website and they wanted me to subscribe to them to get their updated CP, so I went with Forceware.
