as daelmun and senshi have suggested, one of the simplest thing you can do to make things more visible is typing this in your console (hit ~) while you're in game:set gl_modulate 2 [enter]vid_restart[enter]this will make everything quite a bit brighter so you aren't getting lost in the shadows... you should only have to do it once since q2 will save your config when you quit.if this isn't bright enough try a higher number up to 10...
also, you can have any crosshair you want (up to 3 different ones) with regular old quake 2 but they won't be any "bigger" just more visible.
Trust me, the one I attached is bigger than the stock ones, even with the regular q2 client
You might want to find a different client as there is no such thing as "Quake 3.21" - 3.21 is the version number given by default to any client compiled from the source id released, and if it still sports the 3.21 number, chances are it's something a little dodgy . You might want to investigate R1Q2, EGL or AprQ2, 3 of the most popular clients used today.http://www.r1ch.net/stuff/r1q2/http://egl.quakedev.com/http://maniac.aq2world.com/
